COUNTY FORMS

Many of our CPYP counties have developed forms that relate to their permanency efforts. Particularly since CWS/CMS software is limited in the amount of information that it can capture related to family finding, it is very important to get as much information as possible at the time of first contact with a possible connection before that information becomes lost. In addition, tools are needed to help track progress and measure outcomes. The documents listed below are actual forms used by CPYP counties for these tasks. We hope these samples may assist other counties and agencies who are doing similar work.

Relationships/Connections
This form is designed to provide a comprehensive list of all known current and past maternal relatives, paternal relatives, caregivers and other non-relatives such as godparent, friend, teacher, neighbor, etc. such as godparent, friend, teacher, neighbor, etc.

Contact Information
This form is a worksheet to be used while searching for potential connections. Information captured includes dates the potential connection appeared in the file, contact information, and follow up ideas.

Monterey County

Permanency Conference
A permanency conference is a meeting for and about youth in long-term foster care. The meeting is youth-driven and is held to help youth think about their short-term and long-term goals. Monterey County has developed a brochure which describes what a permanency conference is as well as a policy and procedures document for social workers.

YEAH! (Youth Excellence, Achievements and Happenings)
This form is a tool to celebrate and document success in the life of a foster youth. Items can include possible permanent connections, a positive incident or attribute, or individual achievements.

Sacramento County

Father Information
This is a form that Sacramento is using with new cases to gather important family information. They have a comparable form for the mother.

Permanency Review
Sacramento has worked hard to implement permanency system wide. This form is completed and becomes part of the court report so the court is now much more active in reviewing permanency. It can also be used for case review.

Rebekah Children's Services

Outcome Measures Tool
Located in Gilroy, CA, this agency offers children and their families an array of campus-based and community-based care. Permanency Services consist of adoption and foster care, as well as Links 2 Permanency that identifies relatives and others with whom youth will have connections that last a lifetime. This outcomes tool is used by the agency to track permanency measures.

Sonoma County

Internet Search Form
Many counties use Internet search engines such as US Search and Accurint for their family finding work. Sonoma uses this form to initiate an Internet search.

Adolescent Permanency
This instrument can be used as a pre- and post-permanency training tool.

Search Tool
This form is for youth to use to initiate a search for family members. The youth fills it out and gives it to her/his social worker, probation officer, or ILP worker.
